For those patients that are assigned to our medical group on an HMO or AHCCCS plan, we verify your eligibility before each appointment. A copay is expected prior to your visit with the providers.

If you do not have insurance or your insurance company is one that we are not contracted with, there will be a charge of $72.00 prior to being seen. We will bill you for the balance of the charges if any, depending on the complexity of your visit. (Office visit charges vary due to complexity in diagnosis, injections and medications given and or prescribed).

If you have insurance we expect payment on any balance that is remaining after the insurance company has processed your claim. If the balance is not paid within 60 days, we will forward the claim to our collection department.

We do have a policy to bill for patients that “no-show” for their appointment.  If a patient does not call ahead of time to cancel an appointment then the patient will be billed their copay amount.  If there is not an assigned copay amount, the patient will be billed a $10.00 fee.
